Elevating Rural Healthcare

Efficiency | Effectiveness | Empathy

A Model for Rural Health Innovation & Redesign

PROBLEM

SOLUTION

Healthcare costs in rural areas run higher than costs in more urban settings.

Focus on underserved areas with a shortage of providers.

Low provider to population rates
leads to high provider burnout.

Establish Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs to improve patient wellness.

Instances of chronic disease such as Diabetes, Heart Disease, and COPD run significantly  higher in rural areas than in urban areas.

Invest in the necessary physical and digital infrastructure to improve patient outcomes.
